Terms and Conditions for Avant-Garde Center for Wellness Inc

1. Acceptance of Terms By accessing and using the services provided by Avant-Garde Center for Wellness, Inc you agree to comply with and be bound by these Terms and Conditions. If you do not agree, please refrain from using our services.

2. Services Provided Avant-Garde Center for Wellness, Inc offers mental health counseling, therapy, and related support services. Services may be provided in person, via telehealth, or other agreed-upon methods.

3. Appointment Policy

· Appointments are scheduled in advance.

· Cancellations require notice at least 10 hours prior to appointment time. Failure to provide notice may result in a cancellation fee.

4. Payment and Billing

· Payment is due at the time of serviceunless prior arrangements have been made.

· We accept cash/credit/debit

· Insurance claims will be processed as a courtesy; however, you are responsible for any unpaid balances.

5. Confidentiality Your privacy is important to us. All personal and health information is handled according to our Privacy Policy and applicable laws.

6. Code of Conduct Clients are expected to:

· Treat staff and other clients with respect.

· Attend appointments on time.

· Refrain from disruptive behavior.

7. Limitation of Liability Avant-Garde Center for Wellness, Inc is not liable for any indirect, incidental, or consequential damages resulting from the use of our services.

8. Emergency Situations Avant-Garde Center for Wellness Inc does not provide emergency services. In case of a mental health emergency, please call 988, 11 or go to the nearest emergency room.

9. Changes to Terms Avant-Garde Center for Wellness, Inc reserves the right to modify these Terms and Conditions at any time. Continued use of our services after changes implies acceptance of the updated terms.

10. Governing Law These Terms and Conditions are governed by the laws of Nevada/USA
